Medication Form Info:

This form is for those students who take daily medication OR allergies that require medical treatment. If you are not taking medicine to camp or do not have serious allergies, you do not need to fill out this form.

ALL MEDICATIONS MUST BE GIVEN TO THE CAMP NURSE. IT IS AGAINST CAMP POLICY FOR STUDENTS TO ADMINISTER THEIR OWN MEDICATION, THIS INCLUDES OVER-THE-COUNTER PAIN RELIEVERS OR OTC ALLERGY MEDICINE.

Place the completed form plus all medications into a seal-able zip-lock style bag. All medications must be in their original packaging and clearly labled. Prescription medication must be in the original pharmacy bottle with the Rx instructions intact. At check in, there will be a table designated for medication collection. The LOBB will give these medicines to the camp nurse upon arrival.

At the end of the week, the camp nurse will give the remaining medicines back to the LOBB. When you pick your student up (after the WSU football game) please locate the booster with the medicine bag and pick up your student's medication. 

THE ONLY EXCEPTION TO THIS POLICY IS ASTHMA INHALERS. STUDENTS MAY CARRY THEIR OWN INHALERS BUT YOU ARE REQUIRED TO FILL OUT A FORM AND PLACE EXTRA INHALERS IN THE BAG.
